Who Is Eligible for KardiaACCESS?

KardiaACCESS is designed for individuals enrolled in Original Medicare (Part B) who have been diagnosed with one or more common health conditions.

Two Qualification Tracks:

1. Early Heart & Metabolic Support (eCKM Track):

  • Diagnosed with High Blood Pressure (Hypertension)
  • AND/OR two or more of the following: High Cholesterol, Prediabetes (A1c 5.7–6.4%), or overweight / obesity.

2. Established Condition Management (CKM Track):

  • Diagnosed with Type 2 Diabetes (A1c ≥ 6.5%)
  • Diagnosed with Chronic Kidney Disease (CKD)
  • Diagnosed with Heart Disease, history of stroke, or circulatory conditions.
